Clinical Services Overview
At the heart of our specialist care service is our multi-disciplinary team of consultants, palliative care nurses, social workers, occupational therapists and counsellors whose joint efforts combine to provide the focused, supportive and effective help we provide in our community.
All our clinical staff have special skills, experience and qualifications required to provide and further develop high quality palliative care.
We support patients from the point of diagnosis of their illness and throughout their illness. As the needs and circumstances of patients, their families and carers can vary and change so the nature and range of help required.
Each patient, their medical condition, family and carer situation is unique. We aim to deliver care and support tailored to meet their particular needs and requirements. Our aim is to help patients and families to cope with the effects that the illness may be having on their lives through the identifying and managing of symptoms.
All our help, services and facilities are provided totally free of charge to patients, their families and carers.
Referral to the hospice is usually arranged by the patients GP, hospital doctor, district nurse, care manager, social worker or the individual patient. Once a referral is received each patient is allocated a named nurse who will arrange a visit to assess needs and coordinate care.
